Minor under Age 16
Each minor shall appear in person and
a. Both parents must appear together and sign the application form in the presence of a Consular officer;
or:
b. One parent appears, signs the application form in the presence of a Consular officer, and submits second parent's notarized Statement of Consent: Issuance of a Passport to a Minor Under Age 16, authorizing passport issuance of the child.
and shall submit ALL of the following:

Current passport;
Two (2) identical, recent, color photographs taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). Please ask the photographer for photos for a U.S. passport, otherwise you may be given the wrong format.
Important: if you do not provide the following, your child's passport application will be delayed: All applications for children under 16 require both parents' or legal guardians' consent. When applying for a child under age 16, the parent (or legal guardian) who is not accompanying the child must submit a notarized statement of consent and a legible photocopy of their passport or ID card. Non-U.S. citizens may submit a legible photocopy of a signed passport or a signed national photo identification card. An original or legible photocopy of the child's birth certificate.
Please remember to complete your child's social security number on the application form.

When traveling, please remember to carry a photocopy of your child's passport in a separate place. In the event of loss or theft, the copy will facilitate issuance of an emergency passport.
